The W Assisted Living Communities is is seeking a compassionate and dedicated Lead Case Manager. We have our eye out for a candidate with a proven track record of successfully coordinating services for residents, while overseeing and managing the Case Manager Team to remain committed to excellence in resident satisfaction. The chosen candidate will have the opportunity to work with a positive and supportive team of healthcare professionals.  Salary will be based on skill, knowledge and experience also during the probationary period.                                          

Job Responsibilities:

• Maintain firm adherence to the resident’s “Bill of Rights”, as they relate to the facility
• To communicate with the Administrator, Case Management Team and resident’s care providers
• Assessments and coordination of admission/re-admission from the community, nursing homes and hospitals
• Coordinate and supervise clinical care for residents
• Maintain clear and concise documentation in accordance with facility requirements

Qualifications/Requirements:

• Prior experience in Long Term Care is a PLUS or 3 years of experience as a Lead Case Manager required
• Previous  Compuware experience is a plus
• Excellent phone skills and ability to work with families/other care providers
• Strong organizational skills, passion and compassion for the population
• Ability to work cooperatively with multidisciplinary team
