The W Assisted Living Management Company is is seeking a compassionate and dedicated Case Manager. We have our eye out for a candidate with a proven track record of successfully coordinating services for residents, while remaining committed to excellence in resident satisfaction. The chosen candidate will have the opportunity to work with positive and supportive team of healthcare professionals.                                             

Job Responsibilities:

  • Maintain firm adherence to the resident’s “Bill of Rights”, as they relate to the facility
  • To maintain meaningful communication with the resident’s care providers
  • Assessments and coordination of admission/re-admission from the community, nursing homes and hospitals
  • Coordinate and supervise clinical care for residents
  • Clear and concise documentation in accordance with facility requirements

Qualifications/Requirements:

  • Prior experience in Long Term Care a PLUS or 2 years’ experience as a Case Manager
  • Previous case management and Compuware experience a plus
  • Excellent phone skills and ability to work with families/other care providers
  • Strong organizational skills, passion and compassion for the population
  • Ability to work cooperatively with multidisciplinary team
  • Bachelor's Degree is preferred but not required
